Referrals can only be accepted from a patient’s own GP.
GPs fill out a simple form and preferably confirm with a telephone call.
Following this and on the same day a member of the team will visit the patient and family at home to introduce ourselves and explain what we can provide along with checking supplies of anticipatory medications available for symptom control. A small information pack is left containing details of how and when to contact us and some other useful telephone numbers.
